30-90 Day Process

Who may qualify: 

Children who need significant help with bathing, dressing, feeding, mobility, supervision, safety, or other medically necessary daily care.

Examples of conditions and needs we commonly screen for:

  • Cerebral palsy

  • Autism with significant safety, supervision, or personal-care needs

  • Seizure disorders / epilepsy

  • Traumatic brain injury

  • Genetic or chromosomal disorders

  • Spina bifida

  • Muscular dystrophy and other neuromuscular disorders

  • Severe developmental disabilities

  • Feeding disorders, including children who need tube feeding or close choking/aspiration supervision

  • Significant mobility impairments or wheelchair dependence

  • Complex respiratory conditions

  • Gastrointestinal disorders requiring ongoing skilled care

  • Children needing wound care, IV therapy, vital-sign monitoring, transfers, bathing, dressing, feeding, toileting, or other substantial assistance with daily living

*A diagnosis by itself does not guarantee GAPP approval. Eligibility is based on the child’s medical and functional needs, the level of care required, and medical necessity.
